| Most forms you will need are listed
here for your convenience. You may download the forms, print them out and
send them to school with your child on Aug. 9th. Free/Reduced lunch forms
and Emergency cards will be sent home on the first day of school. Please be sure
to complete both sides of the emergency card.

*New students must have a Tennessee immunization record before being admitted to North Stewart Elementary. Immunization Records can be transferred from another state at the Stewart County Health Dept. in Dover. |
