Dear Parents,

Emergencies will arise occasionally that will cause us to dismiss school early.  Because of the number of students enrolled in school and the brief notice we receive that school will be dismissed early, your child will not have time to notify you by phone.

In case of an event of this nature, it is essential that we know what you wish to have your child do:

It is necessary we have this information on file at school.  Therefore, please fill out and return this form to us as soon as possible.

Please talk with your child, so that he/she will know what to do and where to go.

If any changes in this information should occur during the school year, please let your child’s teacher or the office know.

********If at any time your child will be riding a bus other than their own you must send a note or call the school by 2:00.  If  we do not get a note or call, your child will be put on their regular bus.
